Hand numbness at night — neck or wrist?

Hand numbness at night is most often caused by carpal tunnel syndrome – the first 3 fingers go numb, and shaking the hand brings relief. Numbness of the whole arm with pain in the neck or shoulder blade, made worse by moving the head, points to pressure on a nerve root in the neck. If the symptoms go away a few minutes after changing position, the cause may simply be the way your arm lies during sleep.

Below we compare the three most common causes in one table, list the warning symptoms and the changes you can make as early as this evening.

In brief

  • Carpal tunnel syndrome affects approx. 3% of adults, more often women.
  • Numbness in the ring and little fingers is more often linked to the ulnar nerve or the neck than to carpal tunnel syndrome.
  • Sudden numbness in the arm with a drooping corner of the mouth or speech problems – call 112.
  • Physiotherapy consultation in Rydułtowy: 180 PLN; Mon–Fri 7:00–20:00, no referral needed, private visits only (not covered by NFZ, the Polish public health fund).

Neck or wrist — how to tell the cause?

The easiest way to tell the cause is by which fingers go numb and by any accompanying pain. Carpal tunnel syndrome usually affects the thumb, index finger, middle finger and part of the ring finger, without neck pain. Pressure on a nerve root in the neck causes numbness along the arm, often with pain in the neck, shoulder blade or shoulder that gets worse when you tilt your head back or turn it.

Both causes can occur at the same time, so self-assessment has its limits. We describe treatment in our post carpal tunnel syndrome – what a physiotherapist can do, and problems of the cervical spine on the page neck pain.

When is hand numbness just your sleeping position?

Your sleeping position may be the cause when numbness appears after lying on your arm, sleeping with your arm under your head or with your elbow sharply bent, and goes away a few minutes after you move the arm. The nerve is then temporarily compressed or stretched. If the numbness comes back every night despite changing position, or also appears during the day, the cause usually lies elsewhere.

Sleeping with a bent elbow compresses the ulnar nerve – the ring and little fingers then go numb. A wrist tucked under your cheek or the pillow increases pressure in the carpal tunnel. A pillow that is too high or too low keeps your neck bent to the side for several hours.

When should you see a doctor first?

Call 112 immediately if numbness or weakness in the arm appears suddenly together with a drooping corner of the mouth, speech or vision problems, or if numbness in the left arm comes with chest pain and shortness of breath. See a doctor before physiotherapy if your hand is getting weaker, you keep dropping things or the muscles at the base of the thumb are wasting.

A visit to a doctor is also needed in the case of:

  • numbness in both hands with clumsiness (e.g. difficulty doing up buttons) and unsteady walking – this may indicate compression of the spinal cord;
  • numbness after an injury or a fall;
  • fever, unexplained weight loss, a history of cancer.

You will find a neurologist who works with the clinic in the Recommended specialists section.

What can you do yourself before your visit?

For wrist-related symptoms, a night splint that keeps the wrist straight helps, as does avoiding sleeping with your hand tucked under the pillow. For neck-related symptoms, choose a pillow that keeps your head in line with your spine, and take breaks from computer work every 30–45 minutes. Sleeping on your back or on your side with your arm straight reduces pressure on the nerves.

  1. On your side: the pillow fills the space between your shoulder and head, your arm lies in front of you, elbow slightly bent, wrist straight.
  2. On your back: a lower pillow, arms along your body or on your stomach, not behind your head.
  3. Avoid sleeping on your stomach with your head turned to the side.

If you work at a desk, the tips in our post neck pain from computer work will be useful.

How does a physiotherapist find out where the numbness comes from?

The physiotherapist starts with a medical history and an examination: they check sensation and strength in individual fingers, reflexes, neck mobility, provocation tests for the wrist and nerve tension tests. On this basis they assess whether the problem comes from the neck or from the wrist or elbow, and whether a medical consultation is needed. A consultation in Rydułtowy costs 180 PLN.

Therapy may include manual techniques for the cervical spine or the wrist, nerve gliding exercises, ergonomics and choosing a sleeping position. The physiotherapist decides on the number of visits individually after the consultation.

Good to know: for a few days before your visit, write down which fingers go numb and at what time. Also bring your EMG or MRI results, if you have them.

Summary

Numbness of the first 3 fingers that wakes you at night and goes away after shaking the hand points more towards carpal tunnel syndrome, while numbness of the arm with neck pain points to the neck. Symptoms that go away after changing position are usually due to how you lie. Sudden numbness with speech problems is a reason to call for help on 112.

Frequently asked questions

Is numbness in both hands at night also carpal tunnel syndrome?

It may be, as carpal tunnel syndrome often affects both hands, usually more so the dominant one. However, numbness in both hands can also be linked to diabetes, vitamin B12 deficiency, thyroid disease or compression of the spinal cord in the neck. If you also notice clumsiness in your hands or problems with walking, see a doctor first.

Which pillow should I choose if my hands go numb at night?

The pillow should keep your head in line with your spine. When sleeping on your side, its height roughly matches the width of your shoulder; when sleeping on your back, it is lower. The shape, e.g. contoured, matters less than height and comfort. Changing your pillow will not, however, relieve pressure on a nerve in the wrist.

Does a night wrist splint help?

In many people with mild or moderate carpal tunnel syndrome, a splint that keeps the wrist straight at night reduces night-time numbness. Guidelines list it among the basic methods of conservative treatment. It is worth choosing the splint with a physiotherapist, as a wrist bent back too far can make symptoms worse.

Will hand numbness go away on its own?

Numbness caused by the position of your arm usually goes away after a few minutes of movement and needs no treatment. If it keeps coming back for several weeks, regularly wakes you at night or comes with weakness in the hand, it is worth finding the cause, because long-term pressure on a nerve can lead to weakness of the thumb muscles.

Do I need a referral to see a physiotherapist about hand numbness?

No. The “W zdrowym ciele” clinic in Rydułtowy sees private patients only, without a contract with the NFZ, so no referral is needed. It is worth bringing the results of any earlier tests, e.g. nerve conduction studies or an MRI of the neck, and a list of the medicines you take. The clinic is open Monday to Friday, 7:00–20:00.
