Facial nerve palsy — what does rehabilitation involve?

Rehabilitation for facial nerve palsy involves learning gentle, symmetrical exercises for the facial expression muscles in front of a mirror, protecting the eye and learning how not to overload the weakened side of the face. It complements medical treatment, which in Bell’s palsy should ideally begin within 72 hours. In most patients facial function returns within a few months.

Below we explain how to tell facial nerve palsy from a stroke, when to start rehabilitation, which exercises are safe and which treatments are better avoided. Other neurological topics are collected in our guide rehabilitation after a stroke.

In brief

  • A sudden drooping corner of the mouth with arm weakness or speech problems suggests a stroke — call 112.
  • Drug treatment for Bell’s palsy is recommended to begin within 72 hours of the onset of symptoms.
  • Full or almost full recovery applies to approx. 70–85% of people with Bell’s palsy.
  • Physiotherapy consultation in Rydułtowy: PLN 180; the clinic offers private visits only (not covered by NFZ, the Polish public health fund), Mon–Fri 7:00–20:00.

What does rehabilitation for facial nerve palsy involve?

Rehabilitation includes assessing facial movement, learning precise, slow facial expression exercises done in front of a mirror, muscle relaxation techniques, and advice on eating, drinking and protecting the eye. The aim is movement that is as symmetrical as possible and preventing synkinesis, i.e. involuntary movement of one part of the face when trying to move another, e.g. the eye closing when you smile.

The facial nerve controls the muscles that raise the eyebrows, close the eye and move the cheek and mouth. The most common form of damage to it is Bell’s palsy, i.e. a peripheral palsy with no identified cause. Less often the cause is shingles, Lyme disease, an injury or surgery — in that case the diagnosis and treatment are decided by a doctor.

A physiotherapist does not treat the nerve itself, but helps the muscles work properly while the nerve regenerates. The patient does much of the work alone at home, which is why precise instructions are so important.

When should you see a doctor first, and how can you tell palsy from a stroke?

Any sudden weakness of the facial muscles first needs a medical assessment. If a drooping corner of the mouth comes with weakness in an arm or leg, or problems with speech, vision or balance, call 112, as it may be a stroke. In a stroke the forehead usually still moves, whereas in peripheral palsy you cannot wrinkle your forehead on the affected side.

Red flags:

  1. A drooping corner of the mouth with limb weakness, speech or vision problems — 112.
  2. Palsy on both sides of the face at the same time — see a doctor urgently.
  3. Blisters in the ear or on the outer ear, severe ear pain — see a doctor (possible shingles).
  4. Symptoms getting gradually worse over many weeks, a lump near the ear or around the jaw — see a doctor.
  5. An eye that cannot be closed, with pain, redness or worsening vision — see a doctor or ophthalmologist.
  6. No improvement at all after several months — see a doctor again.

A neurologist in the area is listed in the Recommended specialists section — the cooperating specialist is Radosław Bienek, MD, PhD.

When should rehabilitation start?

Rehabilitation can start after a medical assessment, often within the first days or weeks. At first, when the muscles are flaccid, the most important things are education, eye protection and gentle, active attempts at movement. When movement starts to return, you work on precision and symmetry. The programme changes in later phases, so it is worth checking progress with a physiotherapist from time to time.

Guidelines do not name a single optimal day to start physiotherapy. In practice, the first visit is mainly to make sure the patient knows what to exercise, how, and what not to do. Further visits help correct technique before habits form that are hard to change, for example clenching the healthy side of the face too hard.

Which facial exercises are safe?

Slow, small facial movements done in front of a mirror, symmetrically on both sides, with the emphasis on quality rather than strength, are safe. The healthy side should not “take over” the movement. The exercises are done in several short sets during the day. The examples below are general and do not replace a programme set after an examination.

  1. Raising the eyebrows. Looking in the mirror, slowly raise both eyebrows as if surprised, hold for 3 seconds, relax. 5–8 repetitions, 2–3 times a day.
  2. Gently closing the eyes. Close your eyes slowly, without squeezing, hold for 3 seconds, open. 5–8 repetitions, 2–3 times a day.
  3. A small closed-mouth smile. Draw the corners of your mouth out to the sides, watching whether the movement is even on both sides. 5–8 repetitions, 2–3 times a day.
  4. “Pursed lips”. Purse your lips as if to whistle, hold for 3 seconds, relax. 5–8 repetitions, 2–3 times a day.

If the pain gets worse or spreads, stop the exercise and consult a physiotherapist. Also stop if, during one movement, another part of the face starts to move involuntarily, and show this to your therapist.

Which treatments are used and what should be avoided?

The basis is exercise and education, supplemented by gentle relaxation techniques for the facial muscles and sometimes kinesio taping. The evidence for electrical stimulation of the facial muscles is limited, and some specialists are concerned that strong, mass stimulation may encourage synkinesis. That is why the physiotherapist decides on any physical therapy treatment individually, after a thorough assessment of the patient.

What to avoid:

  • Exercises at maximum strength and fast, big grimaces — they reinforce asymmetry.
  • Chewing gum for hours and blowing up balloons — they overload the muscles.
  • Firm massage and stretching of the affected side without instruction.
  • Self-treatment with home devices for facial electrical stimulation.

Good to know: bring your discharge summary or the doctor’s recommendations, the names of the medicines you take and photos of your face from the first days of the illness, if you have them, to the consultation — they make it easier to assess progress.

Summary

Rehabilitation for facial nerve palsy is above all precise, gentle exercises in front of a mirror, protecting the eye and avoiding overloading the muscles. Sudden facial weakness always needs a doctor first, and stroke symptoms need a call to 112. A physiotherapy consultation in Rydułtowy costs PLN 180.

Frequently asked questions

How long does it take to recover from Bell’s palsy?

In many people the first improvement appears within a few weeks, and full or almost full recovery usually follows within a few months. Some patients are left with slight asymmetry or synkinesis. The pace depends on factors such as the degree of nerve damage and age, so the prognosis is discussed by a doctor.

How do I protect my eye with facial nerve palsy?

When the eyelid does not close fully, the cornea dries out easily. A doctor or ophthalmologist usually recommends lubricating eye drops during the day, a gel or ointment at night and an eye shield while sleeping. Outdoors, glasses that protect against wind and dust help. Eye pain or redness needs a prompt medical check.

Can I massage my face myself?

Gentle self-massage, e.g. slow, circular movements of the fingertips over the cheek, can be helpful, but the technique is best shown by a physiotherapist. Do not stretch or knead the affected side of the face firmly. If massage causes pain or involuntary movements of other parts of the face, stop and consult your therapist.

Does a cold draught cause facial nerve palsy?

Bell’s palsy is popularly linked to a cold draught, but the exact cause is not known. Inflammation and swelling of the nerve, e.g. during a viral infection, are most often suspected. Whatever the suspected cause, sudden weakness of the facial muscles always needs a medical assessment before rehabilitation begins.

Do I need a referral to see a physiotherapist with facial nerve palsy?

A referral is usually not required for a private visit at the “W zdrowym ciele” clinic. The clinic has no contract with the NFZ, so all visits are paid, and a consultation costs PLN 180. Before starting rehabilitation, however, you need a prior assessment by a doctor, who will rule out other causes of facial weakness.

How many physiotherapy visits are needed?

The physiotherapist decides on the number of visits individually after the consultation. It depends on the severity of the palsy, how quickly movement returns and how the patient manages the exercises at home. The visits are mainly for correcting technique and changing the programme in later phases, while the daily work is done independently in front of a mirror.
